Volunteer Name Tag Request

Please fill out this form if you need a volunteer name tag, or need changes to the one you already have.  Name tags can be found in the Dug Out (downstairs by the nursery) and the Community Center lobby. Please allow one week for us to process your request.

You need to register each ministry year to get a name tag.  If you have not registered yet, please follow this link - Volunteer Registration

To get a photo name tag, you need to:

  • Register for the ministry year
  • Have a valid background check
  • Have attended a Safe Place Training
  • Have a Safe Place Application turned in
